Geriatric Care Management
Information and Referral
- Provides information and referral regarding physical and mental health; home care and long-term care; transportation; housing; insurance; financial, legal, and social services; leisure; employment inquiries; and support groups
Assessment and Consultation
Conducts
assessments to determine older adults’ needs at St.
Martin’s Senior Center or conducts in-home assessments
for residents of northern Virginia- Coordinates community resources to meet individual needs
- Offers consultations to individuals and families
Parish Program Development
- Assists Diocesan parishes with developing senior support services and programs

Wellness
activities such as exercise classes and Tai Chi, health education
and screening, twice-weekly blood pressure monitoring, individual
counseling, and support groups- Education in English as a Second Language (ESL), computer-skill training, and other classes of interest to seniors
- Nutritious lunches
- Recreation and cultural events such as trips to local museums, parks, theaters, and restaurants; holiday parties; professional entertainment; bingo; intergenerational programs; card games; movies; and travelogues
Schedule of Activities
- Monthly schedule of activities available in several languages at the senior center.
Transportation
- Round-trip transportation to the Center available for all participants residing in the west end of Alexandria City

- There are many opportunities for volunteers to come to share their talents.
